Mahrem reads three optional product metafields. Create them once in Settings → Custom data → Products → Add definition:

Then open any product and fill in the new fields at the bottom of the page.
Tip
All three are optional. Without a privacy image the product photo is simply blurred. Without a discreet name the normal title stays visible.
